When placing your order, please include the
following information:

*Your Company Name

*Quantity / Series Number / Size - of product being ordered.

*Full Physical Shipping Address – Fed-Ex / UPS can not deliver to a
PO Box.
  Orders with a PO Box only will be shipped via USPS.

*Customers Phone Number – Although not required for regular ground
shipping, it is suggested that the phone number be included as the carrier will
use this if further delivery information is required.

Sales tax is charged for any order shipped to Wisconsin, U.S.A. residents only,

a retail invoice is needed to process drop ship orders and must be provided before shipping.
